AdventHealth Well 65+ is seeking an experienced Advanced Practice Registered Nurse or join one of our busy, value based care, primary care practices.

AdventHealth Well 65+ offers a multi-disciplinary approach to primary care for members 65 and older, dedicated to providing members “whole person care” for all health and well-being needs in one location.

Job Requirements

At work, you will experience:

  • Provides triage care and treatment to patients under the auspices of a supervising physician
  • Assures timely and accurate entries of medical histories, progress notes and treatment recommendations in the patients’ electronic medical records
  • Provides care in accordance with policies and procedures
  • Practices within scope of practice as delineated by education and training including, but not limited to:
    • Conforms to prescriptive privileges
    • Prescription of narcotics - adheres to organizational guidelines such as no telephonic narcotic refills and adheres to established processes such as verifying agreements present in EHR when prescribing to patients 
  • Refers patients for laboratory, x-ray for needed services that cannot be provided at the practice 
  • Assures that adequate patient health records are maintained and transferred, as required, when patient care is transferred 
  • Participates with supervising physician in a periodic review of the patients’ health records. 
  • Performs in person and telehealth triage patient concerns 
  • May travel up 25-50% of the time 
  • Works closely with all providers to reduce ED Visits and Hospitalizations 
  • Supports triage nurses in practices for medical guidance and/or protocol escalation
